Before you begin

  • You will need to create a free Submittable account or sign in with Google or Facebook credentials to submit to these forms
  • Do not share logins, even with colleagues and partners, as this can cause technical problems. Instead, use Submittable's submission collaboration tool to work on forms together.
  • Submittable works best on Google Chrome, Firefox, and Safari. Internet Explorer is not supported. Please make sure you are using a supported browser.
  • You can save a draft of your work if you would like to finish filling out the form at a later date
  • Before leaving your computer for an extended period, save your work and log out of Submittable entirely. Idling for several hours can cause errors in savings your work when you begin again.
  • If anything changes with the information you submitted, please request to edit the submission, or if you need to make changes to your form, please withdraw your submission and resubmit. Applicants remain responsible for submitting any form by its required due date and time.
  • We will follow-up with you about your submission by email. Please be sure to safelist notification emails from Submittable and check the email you used to sign up for your Submittable account regularly.

About

Submission of this form is required for contracting of your MassWorks infrastructure, pre-development, or small-town road assistance grant. Grantees may be asked to present additional information, meet with EOED staff, and/or modify submitted information after review.

When completing tables, please contact your existing EOED Contract Manager if you think more and/or different line items would be appropriate for your project, such as more specific Project Milestones and/or additional spending categories in the Project Budget. If you currently do not have an EOED Contract Manager, please follow the instructions below for submitting Questions.

Before you begin

All grantees are required to submit the following:

  • State vendor code (e.g., VC1234567891)
  • Project scope, budget, and schedule details
  • Site plan(s)

A site plan must be submitted with this form, regardless of whether the same file was included in your grant application.

  • Submission of this form is required for closeout of any coastal dredging grant awarded through the MA Dredging Program
  • After further review, you may be asked to present additional information, meet with EOED executive staff, and/or modify your submitted information
  • Submission of this form does not constitute approval of a closeout request, nor is EOED under any obligation to approve such a request

Before You Begin

  • Review your last Quarterly Report submission in your Submittable account
  • Prepare the following Project Deliverables for attachment, unless these documents have already been provided to EOED or are not applicable: bid plans, bid tabulation, general construction (GC) contract, price change orders, and post-dredge survey
  • Grantees must submit a Contractor Authorized Signatory Closeout Certification as part of their closeout form. This certification must be signed and dated by the Grantee's Chief Elected or Executive Officer (CEO). A link to download this certification is provided within the form.

  • Grantees may be asked to present additional information, meet with EOED staff, and/or modify submitted information prior to approval of their report
  • Late or incomplete reports may result in delays in grant payment requests

  • Requests must be supported by contractor and/or vendor invoices for actual work completed and/or supplies delivered
  • Grantees may be asked to present additional documentation, meet with EOED staff, and/or modify submitted information prior to approval of their request 
  • Incomplete submissions will require further review and may result in delays in requests

Before You Begin

  • Prepare all contractor and/or vendor invoices for attachment
  • All construction invoices (i.e., contractor pay applications or "pay reqs") must include their corresponding schedule of values 
  • Grantees must submit a Project Budget Spreadsheet as part of their payment request. A download link is provided within the form for first-time payment requests. Otherwise, grantees should use the Project Budget Spreadsheet submitted with their last Payment Request Form. Past submissions can be viewed at any time through your Submittable account.
